=== Cytochrome P450 inhibitors and inducers === The cytochrome P450 enzymes involved in the metabolism of selegiline have not been fully elucidated. CYP2D6 and CYP2C19 metabolizer phenotypes did not significantly affect the pharmacokinetics of selegiline, suggesting that these enzymes are minimally involved in its metabolism and that inhibitors and inducers of these enzymes would not importantly affect its pharmacokinetics. However, although most pharmacokinetic variables were unaffected, overall exposure to selegiline's metabolite levomethamphetamine was 46% higher in CYP2D6 poor metabolizers compared to extensive metabolizers and exposure to its metabolite desmethylselegiline was 68% higher in CYP2C19 poor metabolizers compared to extensive metabolizers. As with the cases of CYP2D6 and CYP2C19, the strong CYP3A4 and CYP3A5 inhibitor itraconazole has minimal impact on the pharmacokinetics of selegiline, suggesting lack of major involvement of this enzyme as well. == Presentation == MFS usually afflicts individuals in their fifth to seventh decades of life although uncommon cases occur in adults outside of this age range In one large study, MFS was diagnosed in individuals 21 to 96 years old (median age 66 years). Most studies have diagnosed MFS slightly more often in men than women but one large study conducted in France found it to be 50% more common in men. Individuals with MFS present with a tumor located in an extremity (77% of cases, usually in the lower extremity), trunk (12% of cases), and head and neck areas (3% of cases). Rarely, these tumors have presented in the breast, heart, paratesticular region (i.e. area inside the scrotum including the epididymis the spermatic cord along with its coverings), eye, bone, liver, or multiple sites concurrently. Primary tumors presenting in the abdominal cavity, retroperitoneum, or pelvis have been diagnosed as MFS but larger studies indicate that these tumors are far more likely to be dedifferentiated liposarcomas. MFS tumors usually develop as painless, slowly enlarging masses in a muscle, skin (usually below the fascia i.e. a sheet of connective tissue, primarily collagen, running beneath the skin), or one of the non-cutaneous areas described above. In one study of 69 FBS cases 36 were <5 cm, 23 were between 5 and 10 cm, and 19 were > 10 cm in diameter with the largest tumor being 27 cm. MFS tumors often infiltrate along vascular and fascial planes, are incompletely removed at surgery, and consequently recur at the surgical site.

==== Elimination ==== Atomoxetine and its metabolites are eliminated mainly via excretion into urine. Less than 3% of atomoxetine is excreted unchanged in urine regardless of CYP2D6 status, indicating extensive metabolism. In CYP2D6 extensive metabolizers, who are considered to have normal CYP2D6 activity, more than 96% of a dose of radiolabeled atomoxetine is excreted in urine within 24 hours and 1 to 2% is excreted in feces. Conversely, in CYP2D6 poor metabolizers, excretion is slower, with only 27% excreted after 24 hours, a majority of radioactivity excreted within 72 hours, and ~144 hours required for full excretion. In addition, only 80% of radioactivity is excreted in urine while 13 to 22% is excreted in feces in CYP2D6 poor metabolizers. The major excreted metabolite of atomoxetine is 4-hydroxyatomoxetine glucuronide, which accounts for >80% of the dose in urine but <17% of the dose in feces. The fractions excreted in urine as 4-hydroxyatomoxetine and 4-hydroxyatomoxetine glucuronide account for 86% of a dose in CYP2D6 extensive metabolizers, but only 40% in CYP2D6 poor metabolizers. CYP2D6 poor metabolizers excrete greater amounts of minor atomoxetine metabolites, namely N-desmethylatomoxetine and 2-hydroxymethylatomoxetine and their conjugates, than extensive metabolizers. The elimination half-life of atomoxetine is 4.5 to 5.3 hours in CYP2D6 extensive metabolizers. However, in CYP2D6 poor metabolizers, the half-life of atomoxetine is 19 to 21.6 hours, or about 4-fold longer in comparison.

The rank structure was unique; black policemen (known colloquially as "Mapolisa") were Constables, Sergeants, Senior Sergeants, Sergeant Majors, and Sub Inspectors. The white police (known colloquially as 'Majoni') ranks began at Patrol Officer (single gold bar on each shoulder), proceeding to Senior Patrol Officer (two gold bars), Section Officer (three gold bars), and thereafter to Inspector, Chief Inspector and commissioned ranks, etc., as per UK police rank structures.
